Lobos and Cougars Permission to Roster Form for 2010-2011 Seasonal Year

Welcome Lobos/Cougars Parents!
The information you are about to provide will be used as contact information for your child's coach, manager and the CSA office. Therefore, the information needs to be as accurate as possible. As you complete this form, please make sure you do the following:

1. Have a printer available
2. Have Adobe Reader Installed on your computer.
3. Print and Sign the Permission to Roster Form
4. Mail or drop-off this form to CSA Office.
5. Include a check for applicable fees ($200 deposit for Lobos) ($270 Fall season club fee for Cougars)
6. Players NEW to the Cougars or Lobos competitive programs MUST include a copy of a birth certificate.
